I don't think it's connected with any particular situation in the game,
at least mine (4-5 times over the years) were all unrelated, but sadly
never early in the game.
I don't consider backup of the save a solution simply because I play
for hours at a time and going back to an old save would mean losing two
of three hours of play every time it happened. You do not only have to
go through it again, but it will play differently as I've gone through a
number of levels in the meantime that will be recreated.This just kills
the game for me.
Angaband has a system that avoids problems likes this. The save is
deleted only if you are actually killed, so if the game crashes when you
were alive, it's still there. That would resolve the problem of having
to make a backup. Its ability to save without exiting game also removes
the problem for people like me who do not feel like going back to a (in
game play terms) very old save. You can just save upon entering any
level. However, it is unlikely any of this can be implemented in ADOM,
but if someone more knowledgeable knows of a way that an external
program could do that, they would be my heroes forever :)
