People don't always know what they can handle or what's good for them. 
You may certainly argue that it's best to assume that others know what's 
best for them or what they are capable of handling and can make these 
choices themselves, but that doesn't change the fact that some people can 
and do make choices that aren't good for them and do things that they 
really can't handle, even though they claim or even believe otherwise. 
There are also people who claim that they enjoy or can handle things that 
they really can't, just because they don't want to admit it for social 
reasons.

So, yes, characters attacking other characters and GMs attacking the PCs 
in all sorts of ways happens quite often, but I think you are fooling 
yourself if you think it's always clear when the players can handle it and

always spoken of and discussed when one or more of the players can't.
