I think you must be using High Quality rather than Ultra Quality.  As
the other poster mentioned, it turns out Ultra is not intended for any
card out today, though I still don't understand why the timedemo gave
me 59 FPS.  Anyway, I found this site:

that gives the max playable settings for various hardware.  Based on
that page, I tried 1600x1200 at HQ, and got 41FPS, and it plays well.

BTW, that's with no AA.  When I set AA at 4X, my FPS went down to 7.
Can't play it, but it's actually fun to watch the timedemo, because
you see more detail when things move that slow.
