Just about possible.  I do not think it can be 6-6 as there is too big a 
risk your opening will lead to a poor contract but it could be 0-7-5-1
shape 
with a poor heart suit that you would not open 3H with, but now your
partner 
has something in hearts 5H may make or it is a good sacrifice.

Not possible - how do you know 4S is making?  How do you know 5H is not 
going for a lot? what makes you think you will be defending?
