It would, if the island actually constrained the city. But Silvanost
is a circular city of a fixed diameter smaller then the width of the
island. Theoretically, if it expanded beyond its fixed perimeter, then
it would be constrained in dimension and shape by the island it is on,
just as Qualinost is. But Silvanost's shape was artificially selected
by the elves, rather then constrained by the geography. This is
illustrated by the observation that, if they had wanted a larger
diameter city, they could have located the city towards the middle of
the island. There is a difference between a selected geometry of the
city, and the rough geometry of a city that has expanded to its maximun
geographical extent, and that difference is relevant to this question.
As a related point, a good example of a city that started as a regular
geometric shape (similar to Silvanost) and then later expanded roughly
into the constraints imposed by geography (similar to Qualinost) is
Palanthas.
